A set of original production used Armadyne medical information forms given to Max (Matt Damon) after his accident in the factory.
In the film it looks like he is presented with one form and then signs it. Probably they are supposed to be different sides of the same form but in reality they are 2 single sided forms. The second form still has double sided tape on the reverse where it was secured to the bed to keep it still while Max signs it. Comes with a COA from Media Rights Capital (MRC) and VIP Auctions. |
